Born and raised in the Belvidere community, Dr. Chapko has spent more than 42 years building lifelong relationships rooted in trust, compassion, and exceptional patient care.
Known for his calm presence, love for dentistry, and commitment to continuing education, he has become a trusted provider to generations of local families — many of whom now bring their own children and grandchildren to see him.
Outside the office, Dr. Chapko is a proud father of two, a classic rock fan, and a passionate mentor to the next generation of providers within the practice.

Dr. Shah brings an energetic, modern perspective to dentistry, combining advanced cosmetic, implant, and restorative care with a strong passion for helping patients feel confident in their smiles.
Known for her detail-oriented approach and eye for aesthetics, she loves creating natural, beautiful results while making patients feel comfortable throughout the entire process. Her leadership, drive, and commitment to continued growth have helped shape the modern experience patients see throughout the practice today.
Outside of dentistry, Dr. Shah enjoys spending time with family, staying active through pickleball, and balancing the fast-paced world of practice ownership with the relationships that matter most to her.

Patel
Dr. Tulsi Patel earned her Bachelor of Science in Biology from the University of Detroit Mercy and went on to receive her dental degree from the University of Michigan School of Dentistry. She is dedicated to providing high-quality, comprehensive care while ensuring every patient feels comfortable, informed, and confident in their treatment. Dr. Patel is an active member of the American Dental Association, Chicago Dental Society, Academy of General Dentistry, Illinois State Dental Society, and the Winnebago County Dental Society. Outside the office, she enjoys traveling, staying active through running and sports, and exploring new restaurants and cuisines!

Khan
Dr. Khan’s passion for dentistry began at a young age, inspired by the impact dental care had on her grandmother’s life. That experience helped shape the compassionate, patient-centered approach she brings to every appointment today.
Known for her gentle presence and calming demeanor, Dr. Khan loves helping patients feel comfortable, informed, and genuinely cared for throughout their visit. She believes building trust and creating positive experiences is just as important as the dentistry itself.
Outside the office, Dr. Khan enjoys traveling, spending time with family, and embracing life as a new mom.

Malhotra
Dr. Ankit Malhotra is passionate about creating a welcoming, patient-focused dental experience centered around trust, comfort, and long-term oral health. He believes every patient deserves compassionate, high-quality care and takes pride in helping patients feel informed, understood, and confident throughout their treatment journey.
Dr. Malhotra earned his Doctorate of Dental Medicine (DMD) from Rutgers School of Dental Medicine in Newark, New Jersey, after completing his Bachelor of Dental Surgery (BDS) at Genesis Institute of Dental Sciences and Research in Punjab, India. He has also completed advanced education in Orofacial Pain and TMD Disorders through Rutgers School of Dental Medicine.
Known for his calm approach and commitment to continuing education, Dr. Malhotra is dedicated to providing thoughtful, comprehensive care tailored to each patient’s individual needs.
